Everyday Insurance Testimonial Consent
Effective as of: 23 November 2020
The testimonial I have provided is a true and accurate reflection of my views and experience as they pertain to the product or service being promoted by Woolworths Group Limited (Woolworths).
I hereby consent for Woolworths:
1. To contact me in relation to my testimonial and survey response.
2. To use the Testimonial in its marketing, advertising or promotional materials in Australia during the period from today’s date to the date 12 months from today (inclusive), including emails to other customers, TV campaigns, catalogues, posters and point of sale material, online at any of its websites and social media platforms, and corporate communications (such as media releases, press releases, shareholder reports and annual reports) (Materials) without remuneration for the purposes of promoting the product or service subject to the Testimonial as described below;
3. To edit, cut down, adapt broadcast and reproduce the Testimonial in the Materials;
4. To use my name, suburb or/and state in any Materials (without limiting the foregoing, Woolworths will not be obliged to use my name or image in any Materials); and to do any act or omission that would otherwise infringe my moral rights in the Testimonial and/or Materials, to the full extent permitted by law.
I acknowledge and agree that the Materials are the property of Woolworths and to the extent applicable, I assign all right, title and interest in the Materials and if necessary will do any acts and provide any documents necessary to give effect to this assignment.
